Racing heart, chest tightening, breathlessness, the sense that something is catastrophically wrong โ€” panic attacks are among the most distressing experiences in medicine, partly because they are so convincingly physical. The first practical question for anyone who has had one is: which doctor do I go to?

First Attack or Chest Pain? Rule Out the Body Before the Mind

Panic attacks mimic several medical emergencies โ€” cardiac events, thyroid dysfunction, hypoglycaemia, arrhythmia. Doctors do not diagnose panic disorder until these are excluded. If this is your first episode, or if you have:

  • Chest pain that radiates to the arm or jaw
  • Breathlessness that does not resolve in a few minutes
  • Fainting or near-fainting
  • A personal or family history of heart or thyroid conditions

โ€ฆgo to a physician or emergency department first. The tests involved are quick: an ECG, a thyroid panel, blood sugar. Once they come back clear, the clinical picture becomes much more straightforward.

If you have had several attacks and a doctor has already confirmed there is no cardiac or thyroid involvement, you can go directly to a psychiatrist or psychologist. This is what most people presenting with panic disorder are doing โ€” their GP has already done the physical workup.

When It Is Panic: Psychiatrist, Psychologist, or Both?

Panic disorder โ€” the clinical diagnosis when attacks are recurrent and the fear of the next attack is affecting daily life โ€” is treated by two professionals who work together:

Psychiatrist โ€” confirms the diagnosis, rules out conditions like generalised anxiety or bipolar disorder that can present with panic, and considers medication when attacks are frequent, severe, or when there is also depression. Clinical psychologist โ€” delivers Cognitive Behavioural Therapy (CBT), which is the first-line, evidence-based treatment for panic disorder. CBT for panic teaches you what is actually happening in your body during an attack (hyperventilation, adrenaline), reduces the catastrophic interpretation of those sensations, and gradually reduces avoidance behaviour โ€” the pattern of avoiding places or situations where attacks have happened. An 8โ€“12 session course is typical, and most people see meaningful improvement within the first four sessions.

Many people see both. The sequence is usually psychiatrist first (for diagnosis), then psychologist for the CBT work, with the psychiatrist monitoring and adjusting any medication.

What to Look For When Choosing a Doctor for Panic Attacks

In India, credentials to verify before committing:

  • Psychiatrist: MBBS + MD Psychiatry, DPM, or DNB Psychiatry; registered with the MCI/NMC
  • Clinical psychologist: M.Phil in Clinical Psychology from an RCI-recognised institution; registered with the Rehabilitation Council of India

For panic specifically, the experience question to ask is whether they are familiar with Interoceptive Exposure โ€” the CBT technique where you deliberately recreate the physical sensations of a panic attack (controlled hyperventilation, spinning) to break the fear-of-the-sensation cycle. This technique has the strongest evidence base for panic disorder and distinguishes a specialist from a generalist.

A red flag is a treatment approach that offers only sedation with no plan for therapy. Medication for panic should support engagement in CBT, not replace it.

What Happens in the First Appointment

The first session with a psychiatrist for panic typically involves:

  1. A clinical history โ€” when attacks started, how many, what triggers them, what makes them worse
  2. A short physical review and confirmation of any medical workup already done
  3. Discussion of the panic cycle: what is happening in the body, why it feels like danger, and what has been tried so far
  4. A plan โ€” therapy alone, medication plus therapy, or a referral to a psychologist for CBT
