What is CBT and how does it work online?

Cognitive Behavioural Therapy is a structured, time-limited approach to mental health treatment built on a simple model: thoughts, feelings, and behaviours are interconnected, and changing unhelpful patterns in any one area shifts the others. In practice, CBT involves:

Identifying automatic thoughts. The therapy begins by mapping the specific thoughts that arise in problem situations — not in general ("I'm anxious") but in precise detail ("When I'm in a meeting and someone asks a question, my first thought is: I'll say something stupid and everyone will notice"). This specificity is what makes CBT actionable.

Evaluating and restructuring. Once automatic thoughts are identified, the therapist works collaboratively with the client to examine the evidence for and against them and develop more balanced, realistic responses. This is not positive thinking — it is logic applied to habitual cognitive patterns.

Behavioural experiments and exposure. For most conditions, CBT includes structured behavioural components — gradual exposure to feared situations for anxiety, behavioural activation for depression, or habit reversal for OCD. These homework assignments between sessions are where much of the therapeutic work happens.

Online delivery translates this model directly: video sessions run identically to in-person; worksheets and thought records are shared digitally; and behavioural experiments happen in the client's real environment — which is often more useful than a simulated clinic setting.

What conditions does online CBT treat?

Cognitive-behavioural therapy has the most extensive evidence base of any psychological therapy. Online CBT is effective for:

Anxiety disorders. Generalised anxiety, panic disorder, social anxiety, health anxiety, and specific phobias all respond strongly to CBT. Exposure-based components are particularly effective and deliver well online.

Depression. CBT for depression focuses on behavioural activation (counteracting the withdrawal that maintains depression) and addressing the cognitive patterns — self-criticism, hopelessness, all-or-nothing thinking — that sustain low mood. Multiple meta-analyses find online CBT for depression equivalent to face-to-face.

OCD. For OCD, the specific CBT variant is Exposure and Response Prevention (ERP) — a highly structured approach involving graduated exposure to obsessional triggers while preventing the compulsion. ERP requires a therapist trained specifically in this protocol, not just general CBT.

PTSD and trauma. Trauma-focused CBT and EMDR (a related approach) are both deliverable online and have strong evidence bases for post-traumatic presentations.

What happens in an online CBT session in India?

How to find a certified CBT therapist online in India

In India, psychologists are regulated by the Rehabilitation Council of India (RCI). An RCI-registered clinical psychologist with CBT training is the appropriate professional for online CBT. When reviewing profiles, look for:

RCI registration. Visible on the therapist's profile or verifiable on the RCI portal. This confirms the minimum required clinical training (typically an MPhil in Clinical Psychology).

CBT-specific training. Ask directly whether the therapist has completed a structured CBT training programme — not just exposure to CBT techniques during their postgraduate degree. For OCD presentations, ask specifically about ERP training.

Experience with your condition. A therapist who has worked extensively with anxiety will structure sessions differently than one whose primary experience is with depression. Specialist experience matters.
