Intellectual disability (ID) is a neurodevelopmental condition characterised by significant limitations in both intellectual functioning and the practical skills needed for daily life. It is not a single condition but a profile with varying causes, severity levels, and support needs. Early identification and appropriate support significantly improve independence and quality of life. What Is Intellectual Disability?

The DSM-5 defines intellectual disability by three criteria, all of which must be present. First, significant limitations in intellectual functioning โ€” typically an IQ score approximately two standard deviations or more below the population mean (approximately 70 or below), with significant limitations in reasoning, problem-solving, planning, abstract thinking, and learning from experience. Second, significant limitations in adaptive functioning โ€” the conceptual, social, and practical skills needed for daily living across community, school, work, and home contexts. Third, onset during the developmental period, before age 18.

Critically, IQ score alone does not determine the diagnosis or its functional impact. Adaptive functioning โ€” how effectively a person manages real-world demands โ€” is equally weighted and clinically more informative for treatment planning.

Intellectual disability exists on a spectrum of severity. Mild ID (IQ 50โ€“70) accounts for approximately 85% of cases; many individuals with mild ID live semi-independently or independently with appropriate support. Moderate ID (IQ 35โ€“50) involves greater support needs for daily living and educational achievement. Severe and profound ID (IQ below 35) involves significant support needs across most domains of daily functioning.

Symptoms of Intellectual Disability by Domain

Conceptual Skills

Difficulties with language development, reading, writing, mathematics, time management, and understanding of concepts like money, number, and cause-effect relationships. In mild ID, these difficulties may be noticeable primarily in academic contexts; in more severe presentations, they affect functional communication and basic daily living.

Social Skills

Difficulties understanding social conventions, reading social cues, regulating social communication, and forming and maintaining peer relationships. Children with intellectual disability are at higher risk of social victimisation and bullying due to these difficulties, making social skills development and peer education important components of support.

Practical Skills

Difficulties with self-care (hygiene, dressing, meal preparation), managing schedules and routines, using transportation, managing finances, and occupational functioning. These practical skill limitations โ€” rather than cognitive test scores โ€” most directly determine the level of support a person requires and the degree of independence achievable.

Developmental Signs in Infants and Toddlers

In younger children, early signs may include delayed motor milestones (sitting, walking), delayed speech and language development, limited interest in social interaction, and difficulty with learning through play. These signs warrant early developmental assessment โ€” the earlier intervention begins, the greater the benefit from the brain's neuroplasticity.

Causes of Intellectual Disability

Intellectual disability has diverse causes, and in some cases no identifiable cause is found despite comprehensive investigation.

Genetic causes account for a significant proportion, including chromosomal abnormalities (Down syndrome โ€” trisomy 21 โ€” being the most common; Fragile X syndrome โ€” the most common inherited cause), single-gene mutations, and copy-number variants identified through genetic testing.

Prenatal causes include maternal infections during pregnancy (rubella, cytomegalovirus, toxoplasmosis), foetal alcohol syndrome, prenatal exposure to toxins or medications, and severe maternal malnutrition. These causes are largely preventable with appropriate prenatal care.

Perinatal causes include birth complications involving oxygen deprivation (hypoxic-ischaemic encephalopathy), prematurity and low birth weight, and infections in the newborn period.

Postnatal causes include central nervous system infections (meningitis, encephalitis) in early childhood, severe head trauma, lead or other heavy metal poisoning, and extreme early deprivation.

Co-Occurring Conditions

Intellectual disability frequently co-occurs with other neurodevelopmental and psychiatric conditions. ADHD is found in approximately 2โ€“3 times the general population rate. Autism spectrum disorder co-occurs in a significant proportion, particularly at higher severity levels. Epilepsy is more common than in the general population, particularly with moderate to severe ID. Anxiety disorders and mood disorders also occur at elevated rates.

Identifying and treating co-occurring conditions is essential: an untreated anxiety disorder, for example, significantly impairs the adaptive functioning and learning capacity that intervention programmes are designed to build. CBT adapted for intellectual disability can effectively address anxiety and behavioural difficulties.

Supporting a Child with Intellectual Disability in India

In India, children with intellectual disability have legal protection under the Rights of Persons with Disabilities Act (RPwD Act, 2016), which mandates inclusive education and reasonable accommodations. The Rehabilitation Council of India (RCI) provides guidelines for special educators and multidisciplinary teams working with children with ID.

Practical steps for families include: pursuing early assessment with a developmental paediatrician, child psychiatrist, or clinical psychologist; engaging speech therapy, occupational therapy, and developmental support as early as possible; working with the school to develop an Individual Education Plan (IEP) with specific, measurable goals; addressing co-occurring medical conditions (epilepsy, sensory processing difficulties, anxiety); and connecting with parent support groups.

Family involvement is one of the strongest predictors of outcome. The skills and routines developed in therapy transfer most effectively when reinforced consistently at home and across settings.
